This Good Faith Estimate (GFE) shows the costs of items and services that are reasonably expected based on either the scheduled appointment or the codes chosen when generating the estimate. The GFE is based on information known at the time the estimate was created. The GFE does not include any unknown or unexpected costs that may arise during treatment. Charges could be more if complications or special circumstances occur. This GFE is valid for the date of service or 30 days from the date the estimate was generated. This estimate is not a contract and does not require the items or services to be provided by UK HealthCare. Your provider may recommend additional items or services that must be scheduled or requested separately and are not reflected in this GFE. Separate estimates for additional items and/or services will be issued upon scheduling or request. To request a GFE for future items or services, please call the financial counseling line toll free at 855-211-4707, Monday-Friday, 8am to 4:30pm. If you receive a bill for at least $400 more than the GFE for the same items or services, you have the right to dispute the bill by contacting UK HealthCare Customer Service toll free at 800-967-2131, Monday-Friday, 8am to 4:30pm. You may also start a dispute resolution process with the U.S. Department of Health and Human Services, which will not impact the high-quality care provided by UK HealthCare.
